'A clear path to care' is an educational film developed by SA Health for the purpose of training clinical staff about Advance Care Directives and the Resuscitation Plan 7 Step Pathway. This DVD follows a patient's journey from admission through to discharge illustrating at each key touch point best practice for following a patient's values and wishes and how these are taken into consideration when developing clinical instructions for care.
