アイスピック型のニキビ痕は直径が相対的に小さく深いためCO2レーザーなどの治療に抵抗性であり、Punch Techniquesなどの治療法が特に適しています。
本動画では相対的に小さめで深めのニキビ痕を対象にしています。
Punch Techniquesは大別するとパンチエクシジョン(パンチで打ち抜いて閉創)、パンチエレベーション(パンチで打ち抜いて持ち上げる)、パンチグラフティングパンチ部分を自家移植などに分類されます。
それぞれ長所短所・ニキビ痕の型に対する適応の違いなどがありますが、中でもパンチエクシジョンは比較的シンプルな方法であることは大きなメリットです。
但しパンチエクシジョンには比較的口径の小さなニキビ痕に限られる、2次的な創傷治癒創が生じうる、術後抜糸までの間、1週間程度糸を顔面に付けておく必要がある当院の場合といったデメリットもあります。
閉創は張力の強い方向弱い方向を判断し原則的には張力がかからないように閉創します。
それは必ずしも皮膚割線 Langer's cleavage lineとは一致しません。
7~10日程度で抜糸を行いますが、患者様の御来院が困難な場合はそれなりの対策を講じます。
パンチエクシジョンは通常は単独ではなくレーザー、他の注入療法などと同時に行われます。
強度のCO2フラクショナルレーザーによるスキンリサーフェシングなどはパンチ部分の創傷治癒にはマイナスに働きます。従って併用療法は比較的軽めに行います。
