When asked to identify the positive features of ADHD, one of the most common answers offered by individuals is “high energy”. Yet fatigue is one of the most common complains of those with ADHD. Can these opposing sentiments be reconciled?

Fatigue is distinct from sleepiness/drowsiness/sedation although the two often cooccur. Fatigue is the feeling of decreased energy so that one can’t perform to the best of one’s ability. Researchers have devised objective ways to measure both peripheral (muscle) fatigue, and central (brain fatigue). Serotonin, tryptophan, and dopamine all play roles in the perception of fatigue.

With many tasks, those with ADHD may be activating a greater number of brain regions than those without ADHD. This may cause depletion of energy resources, contributing to fatigue. Stimulants, and many other ADHD medications can alleviate fatigue. Ongoing studies suggest branched chain amino acids may be another way to alleviate fatigue in those with ADHD.
