Air Canada 143 was a commercial passenger flight from Montreal, Quebec to Edmonton, Alberta with a stopover in Ottawa, Ontario on July 23rd, 1983
carrying 69 people. The plane was a 767200 registered CGAUN.

Incident:

While the plane was flying at 41,000 feet over Red Lake, Ontario an alarm went off indicating that there was low fuel pressure in the left wing, the pilots turned off the fuel pump in the left wing but the warnings went on until the left engine shut off, the pilots attempted to divert to Winnipeg, Manitoba but the right engine shut off 5 minutes later. The Captain was an experienced fighter and glider pilot so he instead diverted to the Gimli airfield he was previously stationed at, the landing was successful and everyone survived.

Investigation:

There was previously a problem with the fuel gauges on the 767 so fuel had to be measured manually, the plane's fuel was measured in kilograms but the ground crew accidentally measured the fuel in pounds and because pounds weigh less than kilograms the plane didn't have enough fuel.


Atlas Air 3591 was a scheduled cargo flight from Miami, Florida to Houston, Texas on February 23rd, 2019 operated by 3 crew members. The plane was a 767300BCF registered N1217A.

Crash:

The plane was about 50 km southeast of Houston when the First Officer proclaimed the aircraft was stalling and pitched the nose down while increasing power, he was soon stopped by the Captain who pulled up but It was too late and the plane crashed into Trinity Bay at 12:44 PM killing all 3 crew members.

Investigation:

The Goaround mode was activated inadvertently by the Captain after his watch had most likely bumped the Goaround lever, the plane was flying in dense clouds and the First Officer experienced a Somatogravic illusion leading to him pushing down extremely hard, he was also very poor at piloting the aircraft as said in previous reports.


US Airways 1549 was a commercial passenger flight from New York City, New York to Seattle, Washington with a stopover in Charlotte, North Carolina on January 15th, 2009 carrying 155 people. The plane was an A320200 registered N106US.

Incident:

At 3:27 PM 2 minutes after taking off the plane struck a flock of Canadian Geese which caused the engines to shut down, the plane wasn't able to make it back to LaGuardia airport so the pilots made a ditching in the Hudson, the ditching was successful and everyone survived with minimal injuries.

Investigation:

Because the plane was so close to the ground the pilots were unable to review the dual engine failure checklist, attempting to return to LaGuardia might have been successful but not likely and it was concluded the pilots had made the right call.


Pan Am 103 was a commercial passenger flight from Frankfurt, Germany to Detriot, Michigan with stopovers in London, United Kingdom, and New York City, New York on December 21st, 1988 carrying 259 people. The plane was a 747100 registered N739PA and was nicknamed "Clipper Maid of the Seas".

Crash:

The plane was flying over the town of Castlemilk in Scotland when at 7:02:50 an explosion occurred in the forward left cargo bay, the explosion bore a 50 cm hole in the side of the plane and the explosive decompression ripped the skin off of the plane, the lounge section on the second level disintegrated and the structural failure caused the nose (which included the cockpit ad firstclass section) to bend down and break off from the rest of the fuselage taking the No.3 engine with it. The rest of the plane broke up with the midsection of the fuselage and part of the wings crashing into the town of Lockerbie, Scotland killing 11 people, everyone on the plane was killed with the final death toll reaching 270 people.


Investigation:

The plot was most likely conducted by Iran as payback for the shootdown of Iran Air 655 in July of 1988, the Iranians hired a Libyan named AlMegrahi to plant the bomb using Semtex, Megrahi was later caught and imprisoned. Pan Am had failed to prevent the bomb from being placed in the cargo hold, and the company was charged with 19 counts of willful misconduct.


Delta 723 was a commercial passenger flight from Burlington, Vermont to Boston, Massachusetts with a stopover in Manchester, New Hampshire on July 31st, 1973 carrying 89 people. The plane was a Dc9 registered N975NE.

Crash:

The plane was trying to land on runway 4R at around 11 AM but it drifted off the ILS vector and descended right into a seawall 160 feet short of the runway, the plane exploded into flames and broke up into multiple pieces, there were 2 survivors but both would die leaving everyone dead.

Investigation:

The crew was supposed to receive clearance to land 1 minute earlier but the controller busy trying to prevent a collision between 2 other planes so he forgot about Delta 723, when the plane was finally given clearance it's descent rate and speed was too high and the crew failed to make a Goaround which led the plane to crash.
