China Airlines 140 was a commercial passenger flight from Taipei, Taiwan to Nagoya, Japan on April 26th, 1994 carrying 271 people.

Crash:

As the plane was trying to land in Nagoya the First Officer inadvertently selected the Goaround setting, this activated the autopilot which pitched the plane up and increased thrust, the pilots attempted to push the nose down and reduce thrust but this only caused the plane to react more violently causing it to pitch up more, the pilots increased thrust which pushed the nose up so high the plane stalled and crashed just short of the runway killing 264 of the 271 people on board.

Investigation:

The pilots were inexperienced with the A300 and were never trained for a situation like the one they faced, Airbus released a statement 9 months earlier recommending airlines to make modifications to the autopilot allowing it to be disengaged easier but China Airlines never did this and said it wasn't an urgent change.

China Airlines 676 was a commercial passenger flight from Denpasar, Indonesia to Taipei, Taiwan on February 16th, 1998 carrying 196 people.

Crash:

The plane was 1,000 feet above the glideslope when landing at Taipei so the pilots decided to Goaround, while this happened the plane pitched up 42 degrees and stalled into a residential area killing all 196 onboard and 7 people on the ground, 203 fatalities total.

Investigation:

Before the crew decided to Goaround the autopilot was accidentally disconnected, so when the pilots tried to goaround nothing happened and the plane was under no one's control for 11 seconds causing the nose to pitch up and the plane to stall.

China Airlines 611 was a commercial passenger flight from Taipei, Taiwan to Hong Kong on May 25th, 2002 carrying 225 people.

Crash:

25 minutes after takeoff the tail separated from the fuselage, this immediately threw the plane into a nosedive which caused the plane to disintegrated over the ocean killing all on board.

Investigation:

The aircraft had suffered tailstrike 22 years earlier in 1980, instead of replacing the damaged skin, engineers just put a sheet of metal over the damaged area, after 22 years of fatigue cracking the tail section failed and ripped off the plane.

China Airlines 204 was a commercial passenger flight from Xincheng, Taiwan to Taipei, Taiwan on October 26th, 1989 carrying 54 people.

Crash:

10 minutes after taking off from Xincheng at an altitude of 7,000 feet, the plane crashed into a mountain that was part of the Chiashan mountain range killing all on board.

Investigation:

The inexperienced First Officer had taken off from the wrong runway so when the crew was supposed to turn left towards the ocean, they turned right towards the mountains, the ATC and ground handlers also failed to spot the mistake.

China Airlines 120 was a commercial passenger flight from Taipei, Taiwan to Naha, Japan on August 20th, 2007 carrying 165 people.

Accident:

8 minutes after the plane landed, a fire broke out under the right wing, an emergency evacuation was ordered and everyone on board the plane escaped alive.

Investigation:

A piece of the No.5 slat track broke off while the slats were being retracted, this piece of metal punctured the rightwing fuel tank which caused a fuel leak, this fuel leaked down onto the engine exhaust causing the fuel to catch fire.

China Airlines Cargo 358 was a scheduled cargo flight from Taipei, Taiwan to Anchorage, Alaska on December 21st, 1991 operated by 5 crew members.

Crash:

Just a few minutes after taking off the crew reported problems with the No.2 engine and requested to turn back to Taipei, the request was granted and the crew was able to turn back with difficulties, 5 minutes later the plane crashed into a hill right wing low near Wanli, Taiwan.

Investigation:

Engine No.3 had separated from the plane tearing off engine No.4 in the process, Engine No.3 had separated because the front wing spar had broke, after this crash and the crash of El Al 1862 Boeing improved its engine pylons for the 747.

China Airlines 2265 was a commercial passenger flight from Taipei, Taiwan to Penghu, Taiwan on February 16th, 1986 carrying 13 people.

Crash:

After the plane touched down the pilots felt a vibration in the cockpit so they decided to goaround, during the goaround the plane crashed into the sea killing all on board.

Investigation:

The vibration was caused by a burst tire on the nose gear, it is not known why the plane crashed during the goaround but it was most likely due to spatial disorientation.
