We go back to the deep sea to dive once again into heart failure! This is part 3 of a 3part series, we’ll explore the diagnostic approach to heart failure, along with what should be on your differential if a patient presents with dyspnea with exertion. We begin with brain natriuretic peptide (BNP), which is elevated in patients with left ventricular dysfunction and is the most powerful lab to help make the diagnosis for heart failure. Other crucial tests include ECGs to help determine the underlying cause, stress testing for potential underlying coronary artery disease, and chest Xrays for pulmonary edema and cardiomegaly. A transthoracic echocardiogram serves as a pivotal tool to assess the type of heart failure the patient has, and what’s causing it. We also discuss differential diagnoses such as obstructive lung disease, ischemic heart disease, and anemia.
