This week (1st March) we are launching a new Fetal Surveillance Risk Assessment form. For the first few months, this will be a printed A4 page, but once the Intrapartum Booklet is updated, this will be included in the booklet.

This form should be utilised during the intrapartum period for every patient, and then utilised again if a patient’s risk factors change, and at handover to another member of staff. The assessment highlights antenatal and intrapartum risk factors, but is not an exhaustive list. Further information may be found within the relevant guidelines on the intranet. Please ensure that every patient has this risk assessment form completed and filed in their notes.
