It usually happens seruptiously. A manager sees that an employee misses a deadline, asks lots of questions, loses a client. All of a sudden, the manager begins the "set up to fail" syndrome by micromanaging, over criticizing, or withdrawing contact. Whether intentional or not, managers are often complicit in the success or failure of their employees. How do we fix that???
