The US Navy had many technical problems with their torpedo technology during WWII. Even though improvements were made, there were still serious deficiencies which caused duds and misfires.

The USS Tang (SS306) had the unfortunate distinction of being sunk by it’s last torpedo. Ironically, the Tang’s 5th war patrol had been one of its most successful. Lieutenant Commander Richard O’Kane was awarded the Medal of Honor for his actions and Tang and crew were awarded many citations.
