How to File an Eviction Complaint in Illinois

Here is a quick guide on how to fill out an Eviction Complaint in Illinois. Please note that this video is for educational purposes only and should not be a substitute for legal advice.

If you are a landlord ending a lease and the occupants refuse to leave the property after their lease has ended, you ended the lease because they violated the terms of the agreement, or they were trespassers, and they entered unlawfully, use this form. If you are the landlord of a tenant who has not paid rent, five days' notice is required. Be sure to name all unknown occupants because if the Sheriff shows up at the residence to evict people on the property and finds someone on the property not named, the Sheriff will not evict that person. You must attach a copy of the notice or demand of eviction, an affidavit of proof of service of a demand or notice, and relevant parts of the written lease if your eviction complaint is based on a lease violation. If you do not want to renew your lease, you must give 30 days' notice. If the tenant has violated a lease in a different way, ten days' notice is required.
