This is the form I864 Affidavit of Support. It should be filledout or submitted by the US citizen sponsor or lawful Permanent Resident sponsor. I made a sample on how my husband filled out all the pages before I submitted it to USCIS together with the other documents for my Adjustment of Status application... This is based on our own experience and it might be a help for your reference as well.
