1) NEET UG Mark Sheet (True Copy)

ii) XII Std. Mark sheet & Passing Certificate (True Copy) ii) First Professional Exam. Mark Sheet, Passing & Attempt Certificate (True copy)

iv) No Objection Certificate from Relieving College (Original) Annexure C

v) No Objection Certificate from Admitting College (Original) AnnexureD

vi) No Objection Certificate from MUHS Nashik (Original)

vii) Certificate from the Dear/Principal stating that the present college is Recognized by concerned
Council (Original)

viii) Death Certificate of Father/Mother/Supporting Guardian (Death have been occurred after taking admission to 1st MBBS Course)

Ix) Proof of single child of senior citizen (Age of 60 years attained on or before last date of application) alongwith a Notarised Affidavit on Rs. 100/ Stamp Paper stating that He/She is a single child of His/Her Senior Citizen parent

x) Medical Certificate issued by the Medical Board (Original) Annexure E1/E2/E3 or F1/F2. xi) Undertaking of Genuineness of the ground. (Original) Annexure G

xi) Admission letter issued by the Competent Authority/Dean/ Principal xii) Attested photocopy of Caste Certificate

Xii ) Attested photocopy of Caste Validity Certificate

xiii) Attested photocopy of NonCreamy Layer Certificate. xvi) Online Bank Payment Receipt of Rs.15,000/ Or Late fee Rs 25,000/
