HRT vs MHT: why hormone replacement therapy wins over menopausal hormone therapy | Felice Gersh MD

The mainstream medical world is switching from the phrase "Hormone Replacement Therapy" (HRT) to "Menopausal Hormone Therapy" (MHT). This is not just semantics. It is a dramatic shift in the way healthcare providers approach hormones for women in menopause. And this matters.

What is the goal here?

MHT is an all encompassing term and technically, HRT is a form of MHT. In practice, MHT almost always refers to static low dose hormones. That is now considered the normal, mainstream approach to hormones and consequently, it doesn't have a special name. It just is MHT.

For my menopausal patients, my goal is to get as close to hormone REPLACEMENT as possible. There are some patients for whom static low dose hormone therapy is the right option, but most women do best when they take physiologic levels of hormones in a cyclic manner.

By sticking with HRT, I keep my eye on the prize, helping women achieve optimal hormone levels and optimal health.

For me and my patients, HRT means hormone supplementation in perimenopause and full replacement in menopause.
