The Inverted Supinator Test/Sign is part of the cervical myelopathy cluster. Here is a video of what a positive test looks like and I also threw in a positive Hoffman's test while I was at it.

✅ Place the patients pronated forearm over yours and get them to relax as much as possible. Flex the elbow slightly. Use a reflex hammer and tap the distal brachioradialis tendon near the styloid process. A positive test occurs when the fingers flex after the reflex hammer strikes.
✅ A positive test can be an indicator of cervical myelopathy but be sure to add this test to the cervical myelopathy cluster before making a decision.
