Subject: Medical Leave Due to Dental Surgery for 4 days
Dear Sir,
Good day.
This is to inform you that I have to take dental surgery today as per the doctor’s advice. I have been suffering severe pain in my 7 teeth for several months and taking pain killers every day, which are affected so badly to my body. Nowadays my 6 teeth should be removed to fix loose teeth.
The doctor has advised it will take 4 days to set up my 7 teeth. So, requesting you please approve my medical leave from 24th December to 27th December 2022. Will be attending the office on 28th December 2022.
I request you to kindly sanction my leave for those days only so that I can recover my health speedily. Thank you.
