Copies of audited accounts for last 3 years.
Copy of the Memorandum of Association and Rules of the organization.
Land Chitha and Jamabandi Certificate from concerned Revenue authority.
In case of lease holder the copy of registered lease deed for atleast 20 years.
Copy of Plan and estimate of the proposed building and other infrastructures.
Item wise estimate of the project cost.
Fee Structure (item wise and year wise breakup per student per class is to be furnished).
Balance Sheet or Financial status certificate issued by the school management and countersignature by DEEO.
U DISE number and copy of the U Dise acknowledgement.
NOC from the Directorate of Fire & Emergency Services, Assam.
Building safety certificate from PWD (Building) Department of the district concerned.
Health and Hygiene certificate from Joint Director Health Services of district concerned.
Safe drinking water certificate from PHE department.
Individual bio data of and salary certificate of staff issued by the school Management and countersignature by the DEEO.
Declaration by school Management for free admission of special category students as per the RTE Act. 2009.
List of members of Managing Committee with details.
List of furniture or equipments.
Details of play ground with photograph.
Disaster management plan for the school.
Class wise enrolment of students in previous three academic sessions, if applicable.
NOC from Head of the Institutions within 1 Km for Primary and 3 km for middle level (minimum 2 schools).
Scheme of Management as required under section 14 of the Act.
Photograph of Building (3 copies atleast).
