Shalynne McKinney was 22 years old when she died. Shalynne displayed classic symptoms of deep vein thrombosis, a lightly dangerous condition, but one that's also highly treatable if caught in time. She sought treatment at her local hospital but was refused treatment. Days later she died from a pulmonary embolism.

Shalynne's story is an example of a preventable patient death. She is just an example of ONE of the millions who are affected by unsafe care each year. Her death is unacceptable, and that is why we at the Patient Safety Movement Foundation have the audacious goal of ZERO preventable patient deaths.
