1. Affidavit of Support or Form I864 (10 Pages)
2. Federal Income Transcript
3. W2 Forms
4. Marriage Certificate
5. Proof of Domicile
6. Naturalization Certificate or Proof of Naturalization
7. PSA Birth Certificate

Submitted Documents as Applicant:
1. PSA Birth Certificate
2. Biographic Page of Philippine Passport
3. Photographs (Family Pictures)
4. Marriage Certificate of both Petitioners’ and Applicants’
5. Death Certificate of my mother
6. NBI Certificate

Additional Supporting Documents submitted
1. Naturalization Certificate
2. U.S Passport (another form of proof of citizenship )
