O’Briens test is used to evaluate for glenoid labral injuries and AC joint pathology. With the shoulder flexed to 90, the patient internally rotates the arm and pronates the forearm. They then apply an upward force against the examiner's resistance. The patient then externally rotates and supinates the arm and applies an upward force against resistance again. A positive test is pain or clicking while pronated that improves with supination.
