Scapular height neck pain 'shoulders up by the ears'

MSK Neurology

People in forward head posture and scapular depression often feels, falsely, that their shoulders are up by their ears. This couldn't be further from the truth, and therapists are often missing this important element of potential correction.

If the scapula is resting with the superior angle lower than the T2 spinous process, in downward rotation or in excessive anterior tilt (more than 20 degrees), it is almost never elevated. The patient feels like this because their neck is too low and because the rhomboid is too (dysfunctionally) active, desperately attempting to hinder further scapular depression.

Don't be fooled by what they feel. Get out of FHP, get the scapula up and slightly into upward rotation. It'll take a few weeks to get used to, but will feel a lot better, and will take a MASSIVE burden off the neck, as scapular depression promotes FHP and cervical hinge.
