Documents needed by the minor

*Application form signed by both parents.
*Visa requirement checklist make sure to download it
*Cover letter signed by both parents.
*Passport valid for at least 6 months
*Recent passportsize picture
*The minor’s birth certificate from PSA
*If nag aaral Certificate of enrolment and copy ng school ID and if may pasok during the plan trip approve letter of absent from the school
*Travel itinerary of the duration of stay in the Schengen area.
*Health/travel insurance Purchase insurance for your whole period of stay in Europe and make sure that it's valid in all Schengen member states. It should have coverage of at least €30,000
*Travel documents such as plane tickets
*Hotel reservations (print out)

*Parents Married Certificate issued by PSA
*Scanned Front page of the Passport of both parents or legal guardian
*Scanned copy of Parents Bank certificate and bank statement.
*Parents Proof of ties

If traveling without both parents:
*Department of Social Welfare and Development (DSWD Certificate)
*Affidavit of support & consent of parents (especially of the nontraveling parent) or legal
guardian
*Proof of legal custody or Family court order – in cases where only one parent has full custody over the child after dissolved/annulled
*Death Certificate of the other parent issued by the Philippine Statistics Authority (PSA)
on security paper (SECPA)
