Causes are:
Disc bulge or prolapsed or disc protrusion or extrusion L4L5, L5S1
Facet Joint arthropathy
Central canal stenosis or foraminal canal stenosis
Piriformis syndrome

Treatment :
Treatment depends on centralization or peripheralization of pain.
Centralization The pain from leg comes to the centre, means if pain travels down the leg then it comes towards back of thighbuttocksback pain means centralization, it is recovery sign or prognosis

Peripheralization It is bad sign, it means the pain is getting worse from back towards the leg.
