Top 5 Causes of Buttock or Glute Pain

There are many causes of buttock or glute pain. These are the top 5 causes of butt pain (and how to tell them apart) given my experience as a specialist treating thousands of people worldwide with back, butt, and sciatica pain. That literal 'pain in your butt' may not be coming from where you thought it was...

The most common cause of buttock pain is actually REFERRED pain from the back! Discs, joints, ligaments, and other structures in/around the back can refer or send pain signals down into the butt... making folks think that they have Piriformis Syndrome or some other pain in the butt when, in actuality, the back is the root cause. An easy way to tell if the back is the issue, is to move it in various directions... if it reproduces your butt pain, then chances are HIGH your buttock pain is caused by a back problem.

The second most common cause of buttock pain is due to sciatic nerve irritation (sciatica). The most common cause of sciatica is a disc herniation. When nerves are irritated, they cause sharp shooting pain down the leg in a very narrow band. Commonly, pain begins in the butt and travels down the leg into the foot. An easy way to tell if you have sciatica is to stretch the sciatic nerve by performing a straight leg raise test (which I show you in the video). If your leg raise is normal, but you still have a pain in the butt chances are HIGH that it is NOT sciatica but rather something else (keep reading)...

The third most common cause of buttock pain is a hip joint problem. Problems such as hip labral tears or hip osteoarthritis can cause pain and stiffness in the hip. The most common place that folks experience pain when the hip is the source is in the groin. The second most common location of pain is felt deep in the buttock. By moving the hip in a few different directions and performing some basic hip self tests it's easy to tell if your hip is involved in your pain experience.

The fourth cause of buttock pain is due to the piriformis. Some call this Piriformis Syndrome... but, the truth is that pain could be caused by many things deep in the glute. Therefore, some call this Deep Gluteal Syndrome. Either way, to tell if the muscles are the source of pain you can either stretch or contract the muscles of the buttock. If moving the back was fine, stretching the nerve (straight leg raise test) was fine, and wiggling the hip seems okay... but contracting the muscles or stretching the muscles of the buttock reproduces your pain, then you may have Piriformis Syndrome or Deep Gluteal Syndrome.

Finally, hamstring issues can cause pain in the buttock. The location varies slightly, however. Instead of central buttock pain, the pain is localized the the "sits" bone... lower in the buttock, right where the hamstring attaches to your pelvic bone. Again, stretching and contracting it typically hurts.
