Understanding Colchicine Mechanism: Study Breakdown 【USMLE Pharmacology】

Colchicine is used to prevent or treat gout attacks.

Gout is an inflammatory arthritis that is the result of the precipitation of serum urea into crystalized deposits of monosodium urea (MSU) in and around the joint.

These crystals cause sever inflammatory arthritis, which is initiated by mechanisms involving various proinflammatory mediators, such as cytokines and chemokines.


Colchicine affects the molecular pathology of gouty arthritis.
And prevents the formation of microtubules.



Colchicine interferes with the inflammatory molecules found in neutrophils and monocytes.

Colchicine binds to both α and βtubulin to create a tubulin–colchicine complex that prevents the formation of microtubules.

The state of microtubule polymerization can control numerous cellular functions, including
intracellular organelle and vesicle transport

secretion of cytokines and chemokines

and migration, division, and regulation of gene expression.

These actions influence the cell activity known to be involved in inflammatory pathways central to the pathogenesis of gout.

However, with overdose,
Colchicine becomes toxic as an extension of these mechanisms.

The adverse effects and toxicities of colchicine are

gastrointestinal upset
renal failure
neutropenia
damage bone marrow (anemia)
neuromyopathy and rhabdomyolysis
death(accidental and intentional)
