Hospital Acquired Conditions (HACs) are conditions that cause harm to patients but are preventable through highly reliable performance of prevention activities (also known as prevention bundles). Six HACs – adverse drug events, C. difficile infection, central line infection, falls, pressure injury, and surgical site infection – cause the greatest harm to UHN patients.
