"Oral appliances (OA) are an effective treatment for OSA, not only improving
AHI but also a variety of physiologic and behavioural outcomes. Recent comparative effectiveness trials have shown health outcomes between CPAP and OA treatments are equivalent, even in severe OSA, despite greater efficacy of CPAP in reducing AHI. This likely reflects greater nightly adherence to OA compared to CPAP therapy. Recent advances in technologies related to OA treatment have the potential to further improve their efficacy and effectiveness in clinical practice.
Selection of appropriate patients who will respond to OA treatment is an ongoing barrier to use. The now commercially available remotely controlled mandibular positioner offers a
means to predict response from a singlenight mandibular titration study and has shown good positive predictive value in initial testing. The advent of new adherence monitoring technology that can be routinely incorporated into OA devices to objectively monitor treatment usage represents another advance in OSA treatment, which will be beneficial in practice and research. This will further help clarify the role of OA in OSA treatment next to CPAP. Establishing best quality devices that are objectively validated in terms of both efficacy and durability in combination with recent advances in patient selection and treatment monitoring, will continue to optimize OA as an
effective and even firstline treatment for OSA."

"Conclusions
This shortterm study has demonstrated that the health outcomes in
patients with moderate to severe OSA were similar after treatment
with CPAP and MAD. The results are likely explained by the greater
efficacy of CPAP being offset by inferior compliance relative to MAD
resulting in a similar “treatment” AHI with each device. These findings strongly challenge current practice parameters that recommend
that MAD treatment should only be considered in patients with mild
to moderate OSA or in those who have failed or refuse CPAP
treatment. Our findings provide a strong rationale for a longterm
comparative effectiveness study of these two treatment modalities. It
is hoped that such studies will allow a rigorous evidencebased approach to changing current treatment recommendations"

Let's face it, CPAP sucks! It's a pain in the ass but it does work! Unfortunately compliance is very low at around 60%. Mandibular advancement devices like those made by Somnomed have a much higher patient compliance and they are far more convenient and enjoyable to use. They're also equally as effective at treating mild to moderate sleep apnea. They can be considered for severe OSA however the majority of patients have better outcomes with CPAP therapy for severe.
